What to Bring to a DOT Physical

Bring Identification and Current Medical Card
Please bring a valid driver’s license or government-issued photo ID to your DOT physical. If you are renewing an existing DOT medical card, bring your current medical card or any documentation from your previous DOT medical exam, if available.
Bring a Medication List
Bring a current list of all medications you take, including prescription medications, over-the-counter medications, and supplements. If possible, include the medication name, dosage, and how often you take it.
Bring Vision or Hearing Support Items
